Applicants under 18 must provide their date of birth, and the parent or guardian must co-sign the application form.

All applicants must make themselves aware of the Club's Terms and Conditions, Constitution and Privacy Policy. The documents are available on our website or from the Club Secretary; requests can be sent to kznfieldtrialclub@gmail.com. The Secretary is the Club's appointed Information Officer.

PROTECTION OF PERSONAL INFORMATION: The KwaZulu-Natal HPR Field Trial Club respects your privacy and protects the information prescribed by the Protection of Personal Information Act No. 4 of 2013 (POPIA).

In making an Application to the KwaZulu-Natal HPR Field Trial Club for Membership, I attest as follows and agree to comply with the following conditions:

• I understand that the KwaZulu-Natal HPR Field Trial Club's Privacy Policy is available on its website or from its Secretary.
• By making this Application, I consent to my personal details and my dog/s information be held by the KwaZulu-Natal HPR Field Trial Club and electronically on its membership register maintained by the Club. I recognise this is a contractual necessity of membership and in the Club's legitimate interests.
• At all times, I will conform to the Rules & Regulations of the KwaZulu-Natal HPR Field Trial Club as set out in the Club's Constitution, the Terms and Conditions published and any Bylaws, Policies, and Procedures instituted by the Club from time to time. I recognise that Clause 13 of the Club's Constitution authorises the Club Committee to take necessary action should my behaviour or conduct be harmful, prejudicial or injurious to the Club's objects, interest, good order and character.
• I declare that I have never been convicted on a charge of cruelty to or neglect of animals.
• I will pay the Membership Fee and the Annual Membership Renewal Fee as prescribed by the Club's Committee.
• I understand and agree that having submitted this Membership Application voluntarily, the Club is under no obligation to approve and accept it. Kwazulu-Natal HPR Field Trial Club reserves the right to refuse acceptance of any Membership Application. In such cases, the Club will refund the fees paid.
